Tax Form Information: Form 1095-C
Form 1095-C provides information about offers of health insurance coverage made to full-time employees. Employers are no longer required to provide Form 1095-C to an employee unless the employee requests a copy of the form. Forms 1095-C for 2025 have been prepared and are available to applicable employees upon request. Requests will be processed within thirty days of receipt.
